To mail in your completed forms, use certified bonded carrier delivery for mailed applications. (Fed-Ex, UPS, etc.). There are two forms, one for Ecuador and one for the remaining countries. Complete both and make three copies. Mail original and two copies to:

You can also fill out the forms online at http://www.pay.gov and submit them electronically on that site. You will have to set up a login and password first on that site if you have not already done so in the past. Keep one copy for your records.
